Description:


The Transitions of Care (TOC) Clinical Pharmacy Specialist recognizes training that is versed in both inpatient and outpatient/ambulatory care. The TOC Clinical Pharmacy Specialist serves to provide expertise and improved continuity of care in transitioning patients in and out of the hospital setting, recognize and provide interventions to optimize care and eliminate barriers that patients and caregivers face during those transition points by facilitating patient centered care. Leverage their skills for regulatory preparedness, workflow enhancement and continual process improvement while collaborating with other various professions. Demonstrate proficiency in various disease states that are associated with high complexity, high readmission rates and high cost medications. Able to manage own practice efficiently while looking for opportunities to expand services. Provide mentorship and is a clinical resource for staff and work collaboratively within interprofessional team members in Transitions of Care.

Qualifications:


Degrees: Doctorate Licenses & Certifications: Registered Pharmacist Additional Qualifications: Doctorate of Pharmacy (Pharm.D.). If no residency training must have 5 years of practice experience in area of specialty required and Board Certification. PGY1 residency with 2 years practice experience in area of specialty and BPS required. PGY2 residency must have BPS within 1 year of hire. (PGY1=3 years of experience and PGY2=2 additional years). Knowledge of Microsoft office products, including Word, Excel, PowerPoint and other graphics or presentation software. Minimum Required Experience:
